Ask a mid-sized business for its risk register and you will usually get a tidy document: premises, cyber, supply interruption, regulatory change, credit exposure to the largest customer. Each with a likelihood, an impact and an owner. It is a serious piece of work, and it almost never contains the thing most likely to cause a bad quarter, which is that one person holds the knowledge, the relationships or the judgement that the operation cannot run without.
